Enrolment Form<\/h3>\n<p>Download and complete an enrolment application form. <a href=\"https:\/\/www.southernrivercollege.wa.edu.au\/wp-content\/uploads\/2025\/05\/Enrolment-Document-2025.pdf\" target=\"_blank\" rel=\"noopener\">CLICK HERE TO DOWNLOAD.<\/a> Please complete the form in full. Should you need assistance, please contact the Front Administration Office.<\/p>\n<p>&nbsp;<\/p>\n<h3>2. Original Documents<\/h3>\n<p>When enrolling bring the following original documents along with the enrolment form to the Administration Office between 8.00am &#8211; 3.45pm, Monday &#8211; Friday, during term time.<\/p>\n<p><strong>Proof of address<\/strong><\/p>\n<ul>\n<li>\u00a0Primary document &#8211; Most recent shire rate notice or current rental agreement (with a minimum of 12 months tenancy from date of enrolment).<\/li>\n<li>Plus a water, electricity or gas bill as well.<\/li>\n<\/ul>\n<p><strong>Original birth certificate<\/strong><\/p>\n<ul>\n<li>Your child\u2019s original birth certificate.<\/li>\n<\/ul>\n<p>Original documents need to be sighted and as such no email applications will be accepted, unless currently living outside of Western Australia or in a remote region.<\/p>\n<h3>3. Immunisation Records<\/h3>\n<p>The school must sight a recent copy of each student&#8217;s AIR Immunisation History Statement at the time of enrolment. The statement needs to say (up to date) on the top of the statement. Health Conditions<\/h3>\n<p>If you child has a medical or health condition \u2013 please advise the Administration Officer when submitting your enrolment form. This will ensure we have all the correct procedures in place to assist your child. More information can be found on our <a href=\"https:\/\/www.southernrivercollege.wa.edu.au\/index.php\/policies\/toggle-elements-2\/\" target=\"_blank\" rel=\"noopener\">Health &amp; Medication<\/a> page.<\/p>\n<h3>5. Overseas Students<\/h3>\n<p>Students who are not Australian Citizens must present their passport and visa grant notice at the time of enrolment. Some students on visas will be required to enrol through TAFE International WA. Southern River College is currently not able to accommodate international students. <a href=\"https:\/\/www.tafeinternational.wa.edu.au\">https:\/\/www.tafeinternational.wa.edu.au<\/a><\/p>\n<h3>6. Overseas Parents<\/h3>\n<p>If your child is born in Australia but both parents are born overseas, we will require a copy of the child\u2019s Australian passport or parents\u2019 citizenship documents to determine if the child is eligible for Australian citizen. Special Needs<\/h3>\n<p>Parents\/carers of students with special needs are asked to contact the school at the time of enrolment, to discuss their child\u2019s unique requirements. This will ensure that the most appropriate program is put in place to meet the needs of your child.<\/p>\n<h3>8. Student Records \/ Information<\/h3>\n<p>It is important that parents\/carers advise the school immediately if there are changes of address, telephone number, emergency contacts or medical details. This will ensure that the welfare of all students is given the highest priority, especially in times of urgency.<\/p>\n<h3>9. Access Restrictions<\/h3>\n<p>Parents\/carers are reminded that the School Education Act 1999 obliges parents\/carers to inform schools of any family court orders or other orders which are applicable to their children. The school must sight the original documents and all details are treated in the strictest confidence.
